The name was changed to Continuous Care Services in 2000.

Arrangements for Continuous Care Services home care can be made prior to hospital discharge by contacting the Case Management or Social Services department in the hospital. However, it is not a requirement to be hospitalized to receive services. Care will be provided under the supervision of your physician.
